Please note: Pharmacies can carry over existing fax numbers. Here are the step by step instructions. Enroll with DocuSign, please fill out the attached , and have handy a copy of the phone bill that has account info with the fax number. A sales representative will contact you via email for the additional paperwork. Once the application process is completed, it will take 3 to 4 weeks to carry over.
